How to Obtain a Birth Certificate
Certified copies of birth certificates can be obtained through the Winnebago County Health Department for persons born in Winnebago County, Illinois.
These records may be requested by and released to the mother, the father (if listed on the birth certificate), person named on the certificate if over 18 years old, or a legal guardian or representative with supporting court documents. Certified copies may be obtained either in person at the Rockford office, through the mail or you can order and pay on line by clicking here: E-Pay.

Eligibility

A certified copy of the birth certificate may be requested and released to the mother, the father (if listed on the birth certificate), person named on the certificate if over 18 years old, or a legal guardian or representative with supporting court documents

Cost
|
The cost of a certified copy is $12.00. Additional copies of the same certificate that are purchased at the same time are $4.00 per copy.
