How to Obtain a Death Certificate

Certified copies of death certificates can be obtained through the Winnebago County Health Department for those who have passed away in Winnebago County, Illinois.

These records may be requested by and released to the family or funeral home.   Certified copies may be obtained either in person at the Rockford office, through the mail, or on-line by clicking here: E-Pay.

Eligibility
A certified copy of a death certificate may be requested and released to a family member or a funeral home.
Cost
The cost of a certified copy is $17.00.  Additional copies of the same certificate purchased at the same time are $7.00 per copy. For all mail or electronic (E-Pay) requests, there will be an additional $3 handling fee. Please include this additional fee with your mail request. This fee will automatically be reflected on E-Pay.
How do I get services?
The following information is required to obtain a death certificate:
  • NAME OF DECEASED
  • DATE OF DEATH
  • SIGNATURE OF REQUESTING PARTY
  • RELATIONSHIP TO DECEASED
  • CURRENT DRIVER'S LICENSE OR STATE ID


To obtain a certified copy of a death certificate, please do one of the following:

  • Bring the above information to the Winnebago County Health Department (555 North Court Street)
